We are delighted to accept new patients who live in Bradford on Avon and surroundings areas onto our practice list.


DOES IT MATTER WHERE I LIVE?
Yes. We can only accept patients who live within this geographical area as this allows us to make house calls when necessary and to balance the number of patients between the different practices.

If you wish to register with us, please visit The Health Centre and talk to our reception staff. It would be a great help if you could bring with you your up-to-date medical card. If you cannot find it, do not despair. Simply see our reception staff who will give you a simple form to fill in. Please ensure you know the following details:-

• Full name, including previous names
• Date and place of birth
• Last address
• New address and post code
• Last name and address of your previous Doctor
• NHS number if known - not the same as your National Insurance number!

NEW PATIENT QUESTIONNAIRE
We require all new patients to complete a confidential questionnaire about their medical history. This will help the Doctors and Nurses at The Health Centre to appreciate your health needs.
