CORONARY HEART DISEASE (CHD) CLINIC
WHAT IS THIS CLINIC?
This clinic is run by a nurse who specialises in heart disease, and is for patients with various heart conditions.
HOW WILL I BENEFIT FROM THIS CLINIC?
At the clinic the nurse will discuss blood results with you. She will also check your blood pressure and discuss:
- Smoking and giving up
- Diet and cholesterol
- exercise and lifestyle
- Medication, action and side effects
and allow time for any problems about your condition. She will also make sure you are aware of the pneumonia vaccine, which is recommended and also remind you to have your flu vaccine yearly.
If you have recently had a heart attack or bypass operation, then a rehabilitation programme is available. She will update your medical records on the computer and also your personal booklet if you have one.
HOW DO I ACCESS THIS CLINIC?
You should be sent a letter inviting you to attend yearly, around your birthday. The letter will ask you to make an appointment for a blood test at least 2 weeks beforehand. You may also be referred to the clinic by your GP or cardiac liaison nurse at the hospital.
